چکیده

mm in length (1). This was performed in a large cohort of subjects and includes sites that do not contain significant obstructive disease. Although one segment was studied in each subject, each epicardial coronary artery is reflected in the total cohort (i.e., this is not a study of disease limited to the left main segment). Further, the volumetric approach defines segments by the fixed anatomic presence of arterial side branches and provides a greater opportunity for precise matching and investigation of the factors that influence the natural history of atherosclerosis. Studying atherosclerosis within the left main coronary artery, the investigators found no significant correlation between the level of low density lipoprotein (LDL) cholesterol and atheroma burden (8), in support of our findings. Interestingly, they found a significant correlation between baseline LDL cholesterol and progression of atheroma at the region studied. The degree of correlation was much greater than what has been subsequently been reported for the relationship between the degree of change in LDL cholesterol and atheroma volume in patients treated with a statin (6). Given that atherosclerosis is a complex pathological process that results from the influence of a large number of factors on the arterial wall and that there is a substantial overlap between levels of LDL cholesterol and incidence of cardiovascular disease, it would be surprising to expect anything greater than a mild correlation between these factors at most.

Screening Asymptomatic Diabetics for Coronary Artery Disease using Diabetic Autonomic Neuropathy as Selection Criteria

Diabetics with Autonomic Neuropathy (DAN), or disease of the involuntary nervous system, represent a population at especially high risk for cardiovascular mortality, including death from Myocardial Infarction (heart attack) and Sudden death [2]. They stand to derive a mortality benefit from surgical revascularization (a bypass operation), however due to neuropathy they often do not exhibit symp...

The value of myocardial perfusion imaging with Tc-99m MIBI for the prediction of perfusion improvement after percutaneous transluminal coronary angioplasty

Introduction: Percutaneous transluminal coronary angioplasty (PTCA) is an effective method for revascularizing of stenotic coronary vessels. Lack of response to this treatment, either in symptomatic or asymptomatic patients, is usually due to incomplete revascularization, restenosis, and/or irreversibility of myocardial perfusion. Introduction of a noninvasive metho...

Is there an additional benefit from coronary revascularization in diabetic patients with acute coronary syndromes or stable angina who are already on optimal medical treatment?

Cardiovascular disease (CVD) is common in patients with diabetes mellitus (DM) and related clinical outcomes are worse compared with non-diabetics. The optimal treatment in diabetic patients with coronary heart disease (CHD) is currently not established. We searched MEDLINE (1975-2010) using the key terms diabetes mellitus, coronary heart disease, revascularization, coronary artery bypass, angi...

Prognostic significance of asymptomatic coronary artery disease in patients with diabetes and need for early revascularization therapy.

AIMS Information on the clinical outcome of patients with diabetes with silent myocardial ischaemia is limited. We compared the clinical and angiographic characteristics, and the clinical outcomes of diabetic patients with asymptomatic or symptomatic coronary artery disease (CAD). METHODS Three hundred and ten consecutive diabetic patients with CAD were divided into two groups according to th...

Diabetes and coronary revascularization.

CONTEXT Patients with diabetes mellitus account for approximately 25% of the nearly 1.5 million coronary revascularization procedures performed each year in the United States and experience worse outcomes compared with nondiabetic patients.
